A new canine dermatology study is challenging how much confidence clinicians should place in adhesive tape strip cytology when diagnosing Malassezia overgrowth or Malassezia dermatitis. In the new analysis, 12 examiners evaluated two slide sets: 12 slides paired with photographs of pruritic dogs, where a presumptive diagnosis of Malassezia dermatitis was based on detecting at least one yeast organism, and eight slides paired with photographs of non-pruritic dogs, where positivity required an average of at least one yeast organism per field. Agreement was measured after review of 10, 20, 30, 40, and 50 oil immersion fields. Among pruritic-dog slides, overall interobserver reproducibility was only fair, with a Fleiss’ kappa of 0.379. Among non-pruritic-dog slides, it was moderate, with a Fleiss’ kappa of 0.509. The authors also found that increasing the number of fields reviewed did not solve the problem, and that examining 50 fields already takes about four minutes per slide, which they described as impractical to extend further in routine practice. One interesting nuance is that agreement changed depending on the diagnostic cutoff. Writing about diagnosis and management of Malassezia dermatitis, dermatologist Alice Jeromin, DVM, DACVD, cited task force guidance that surface cytology is useful to determine whether yeast or staphylococci are present at lesional sites, but that antimicrobial treatment decisions based solely on microbe numbers are “incorrect and inappropriate.” She also noted that, because of poor repeatability and reproducibility in counting Malassezia, cytology results may be better limited to reporting the presence or absence of detectable organisms. Tape-strip prep remains attractive because it’s quick, inexpensive, and practical in folds and recessed areas. Still, the evidence suggests clinicians shouldn’t over-interpret a negative result, and they should be cautious about rigid numeric thresholds, especially if those thresholds drive antifungal prescribing.
